I understand that participation in dance classes, rehearsals, competitions, and related activities involves inherent risks including but not limited to falls, collisions, muscle strains, sprains, fractures, or other injuries. I voluntarily assume ass risks associated with participation.

In the event of an emergency, I authorize Dynasty Dance Company and its staff to seek medical treatment for the participant if I can not be reached. I understand that I am financially responsible for any medical expenses incurred.

I hereby release, waive, and hold harmless Dynasty Dance Company, its owners, instructors, staff, and affiliates from any and all liability, claims, or demands arising from participation in studio activities.

All dancers and parents are expected to maintain respectful behavior. The studio reserves the right to dismiss any participant for inappropriate conduct.

$160.00 a month (4 hours a week 4 classes)

$45 a month (Per 1 hour class)

2+ classes = $40 per class/month
